Description

Responsibilities:

  • Combined Support Services:
  • Collaborate with healthcare providers, community programs, and service agencies to coordinate care plans and address clients' needs, including:
    • Assist with obtaining community resources like food banks, housing assistance, and medical care.
    • Facilitate transportation and support with appointment scheduling.
    • Help with applications, benefits forms, and financial resources.
    • Provide education, support, and guidance on healthy living, symptom management, coping mechanisms, and relapse prevention.
    • Assist with crisis management, de-escalation, and resource connections.
    • Participate in creating and implementing client Plans of Care
    • Support clients in developing interpersonal skills, life skills, social networks, and healthy coping strategies.
    • Facilitate connection to substance abuse programs, mental health treatment, and HIV/infectious disease testing and services.
    • Promote a strengths-based approach to empower clients in their recovery and mental health journey.
    • Work with families and support systems to foster understanding and support for the client's needs.
  • Tailored Recovery Support:
    • Work with families and support systems to foster understanding and support for the client's needs.
    • Attend interdisciplinary team meetings.
    • Provide peer support (If certified as a Peer Specialist)
  • Administrative Responsibilities:
    • Thoroughly document client interactions, progress updates, and relevant information within 24hrs. 
    • Attend team meetings and actively participate in care discussions.

Working Conditions and Additional Requirements:

  • Work primarily in community settings, including home visits, but may also involve work at service sites or program facilities.
  • Flexible hours may be required, including some evenings or weekends, to accommodate clients' needs.
  • Adhere to all HIPAA and 42 CFR privacy regulations and ethical guidelines.
  • Compliance with DC mandated vaccinations or qualified exemptions.

Qualifications:

  • Education:
    • High school diploma or equivalent required:
    • Associate or Bachelor's degree in psychology, healthcare, or a related field preferred.

  • Experience:
    • Minimum of 1 year of direct patient/client service in a healthcare or social service organization
    • Experience as an LPN, CNA, Medical Assistant, Recovery Coach, Certified Peer Specialist, or similar roles is highly valued.
  • Licenses/Certifications
    • Must possess a valid driver's license in good standing and access to a vehicle during working hours.
    • CPR/First Aid Certification required.
    • Completion of a Community Health Worker (CHW), Medical Assistant (MA), or other relevant certification program is required for continued employment and strongly preferred upon application.
  • Skills:
    • Proficient with smartphone apps and basic computer applications
    • Strong verbal and written communication abilities
    • Excellent time management and organization skills
